Keep the packet focused

The initial review covers up to 40 relevant pages; a follow-up covers up to 10 new pages. Tell us before sending a larger packet.
Use complete, readable reports, including their dates and conclusions. Do not crop away context.
Avoid duplicate reports, billing statements, insurance cards, and unrelated records.
Do not order new tests just for this service. We explain existing written records.
We do not independently interpret original scan images, angiogram video, or other raw imaging.

Want help obtaining the reports?

For $75, we request agreed reports from up to two practices, follow up, and organize the documents that arrive. You can gather the reports yourself and skip the collection fee.

Bring a medication list

Keep the names and directions from your labels or current records together, including nonprescription medicines and supplements. Mark missing or conflicting details for your prescribing team to confirm.
