Service and scope
Plain Spoken Health offers education and visit preparation based on existing written cardiology records and documented recommendations. It does not provide a clinical second opinion, diagnosis, treatment, prescriptions, monitoring, or emergency care. It does not establish a treating physician–patient relationship. Your treating clinicians direct your care. Nothing in these terms excludes rights or duties that cannot lawfully be excluded.
Eligibility and acceptance
The service is for adults aged 18 or older in New York. We confirm eligibility and scope before accepting a case. A website visit or inquiry does not mean that your case has been accepted. We may request more information through an approved method, narrow the scope with you, or decline cases that do not fit the service. Declined requests are not charged.
The $350 package
- Up to 40 relevant pages of written records
- One main cardiology question
- A 30-minute physician phone discussion
- A plain-language written summary
- Questions to bring to your treating team
- One clarification requested within 7 days of your summary
The service addresses one main cardiology question. It excludes independent interpretation of original imaging, new testing, ongoing messaging, and treatment decisions. A clarification is about the original question; a new question or new records may require a new scope agreed in advance.
More than 40 pages
Tell us before transferring records. We will help identify relevant reports and duplicates. We do not charge automatic overage fees or silently omit necessary records. If the question cannot be addressed within the package, we will agree a narrower scope or decline it. No different service or additional fee is authorized without a separate written agreement.
Timing and payment
There is no fee for an initial inquiry. Payment is requested only after acceptance, confirmation of scope, and agreement on the appointment and delivery target. Targets are an initial response within 2 business days, a call within 5 business days after acceptance, payment, and complete records, and a summary within 2 business days after the call. A clarification requested within 7 calendar days of the summary has a target reply within 2 business days. Business days exclude weekends and U.S. federal holidays and are measured in Eastern Time.
Missing records and your scheduling needs may extend timing. We confirm actual dates before payment and communicate changes. This service must not be used to delay time-sensitive care.
Cancellations, rescheduling, and refunds
- Before the call: cancel before the scheduled start for a full $350 refund, including when record review has begun.
- Missed appointment: reschedule once at no extra charge, or request a full refund before the replacement call starts.
- After a completed call: payment is generally nonrefundable, provided we deliver the included written summary. No particular medical outcome is promised.
- If we cancel or cannot deliver: you receive a full refund.
- If we miss a confirmed call or summary deadline: you may accept a revised date or request a full refund. You do not have to accept a revised date.
- Processing: we initiate approved refunds to the original payment method within 5 business days. Your payment provider controls when the credit appears.
Email hello@plainspokenhealth.com with “Cancellation” or “Refund request” in the subject and enough nonmedical information to identify your booking. Do not include card numbers or health information. Applicable statutory rights are not limited by this policy.
Insurance and account reimbursement
The package is self-pay and we do not submit insurance claims. We can provide an itemized receipt. Insurance reimbursement and FSA/HSA eligibility are not guaranteed; confirm with your plan administrator before payment.
Your records and participation
Only provide records you are entitled to share, using an approved transfer method. Do not email medical records or upload real information to the current test portal. You are responsible for identifying the question, supplying accurate and complete relevant written records, and obtaining permission for other participants. Incomplete information may prevent a useful review.
Website content and use
Public guides and the fictional sample are general education. The sample is not a real case or individualized advice. You may print the checklist and example for personal use. Do not misrepresent a fictional sample as an actual physician assessment, attempt unauthorized access, or interfere with website operation. Outside links are provided for background and do not imply endorsement or affiliation.
Changes and questions
We may update these terms for future requests by posting a revised effective date. Changes do not retroactively reduce the scope or refund rights of an already accepted, paid review. Questions can be sent to hello@plainspokenhealth.com.
