Sending records to Plain Spoken Health

Use the delivery instructions supplied for the specific records request.

Start with the request and signed release

Check the patient identifiers, selected reports, date range, and recipient against the signed authorization. Send only the requested records. If your practice needs its own release or other documentation, tell us before sending.

Use the private upload link

Secure upload is our preferred method for collecting records. We arrange a private link for the practice and the agreed reports. Fax delivery is not currently available. If a practice cannot use an upload link, contact us before sending anything so we can discuss the options.

We confirm that the delivery method is available for your request before collecting the $75 fee. Please wait for your instructions. Do not send medical records or signed releases to our general email inbox.

Use the link supplied with that specific request. It allows delivery of reports without giving access to the patient’s case or other files. If the link has expired or does not work, ask for a replacement. Do not use another patient’s or practice’s link.

Prepare the reports

Send readable, complete PDF reports. Keep reports in date order where practical and avoid duplicate copies. The private delivery instructions give the file-size and page limits. Tell us if the packet will exceed those limits so we can arrange a suitable transfer.

Send signed releases through the separate method agreed for forms. Do not mix authorizations into the medical-report packet.

If your practice needs fax

Fax delivery is not currently available. Contact us to discuss another way to deliver the requested reports. Do not send to an unconfirmed number or attach records to a general email inquiry.

After delivery

A delivery receipt means the file arrived for checking. It does not confirm that the packet is complete, that a physician has reviewed it, or that the review’s 48-hour period has begun. We check the received reports against the request and follow up about missing or unreadable material.
